How to apply?

This programme is exclusively available at no charge to AIA customers who are 40 years old and above and has diabetes, high cholesterol or high blood pressure. Simply be one of the first 1,500 to activate this programme on the Amped app.

  • Register your interest with Amplify Health here.
  • You will receive an email requesting your health screening results from within the last 3 months, which must meet our eligibility criteria.

If you do not have a recent health screening report, Whitecoat provides complimentary basic health screenings for the first 500 customers. Subsequently, it is offered at a preferential rate of S$45 (before GST). 
 
Simply indicate your interest in a health screening from WhiteCoat when registering for the programme with Amplify Health. 

To complement the programme, you could get a Freestyle Libre Sensor at S$10 (U.P S$103.10) from Abbott to help you track glucose levels in real time with zero finger pricks.

The Chronic Disease Management Programme is open to the first 1,500 'activated' customers. The 'activated' customer is defined as an AIA insured customer who has completed the following: 
 
  • Submitted his/her health screening result to Amplify Health and has received confirmation of his/her eligibility for the Chronic Disease Management Programme 
  • Completed a first video or text-based consultation with the coach and have at least 1 personalised health goal set by the coach

The Chronic Disease Management Programme is not a contractual benefit. Amplify Health is an independent third-party company. AIA Singapore Private Limited ("AIA") shall not be responsible or liable in any way for any act, omission, advice or information that is given, or statements made by Amplify Health or other persons (including healthcare providers) arising from or in connection with the provision of the medical services, products, any other value-added services provided by Amplify Health or any marketing activities conducted by Amplify Health. AIA reserves the right, at its sole and absolute discretion, to cancel, terminate and/or withdraw this value-added service any time or for any reason without notice.  
 
The collection, use, disclosure, and retention of your personal data is subject to the AIA Personal Data Policy (Singapore), to which you agree and accept.
