What is Home Palliative Care?

Home palliative care offers essential services for individuals who are terminally ill, allowing them to receive care in the comfort of their own home. Common palliative services include changing of wound dressings, feeding tubes, stoma care, urinary tube changes, provision of supportive medicines and nutritional feeds. 

Who is eligible?

This service is available for insured customers with the following plan(s):
*For AIA HealthShield Gold Max A with either AIA Max VitalHealth A, AIA Max VitalHealth A Value or AIA Max VitalCare 
 
Insureds who are diagnosed with terminal illness (including cancer) with expected survival period of 12 months or less. 

How to apply?

To apply for the service, you need a referral letter from your treating doctor to the AIA Appointed Home Palliative Care Provider(s). 
STEP 1

Insured requires home palliative care service.

STEP 2

Insured/referring treating doctor contacts AIA Appointed Home Palliative Care Provider to submit referral letter*.

STEP 3

AIA Appointed Home Palliative Care Provider contacts the insured.

*The referral letter must include a declaration that the insured is diagnosed with terminal illness (including cancer) with an expected survival period not exceeding 12 months.

AIA Appointed Home Palliative Care Providers ("AHPCP") are independent third-party companies. AIA shall not be responsible or liable in any way for any act, omission, advice or information that is given, or statements made by AHPCP or other persons (including healthcare providers) arising from or in connection with the provision of the medical services, products, any other value-added services provided by AHPCP or any marketing activities conducted by AHPCP.
 
The availability of the Home Palliative Service with AHPCP does not mean that the medical bill(s) will be automatically claimable. Claims eligibility will be subject to the terms and conditions of the AIA HealthShield Gold Max plan.
 
Collection and Use of Personal Data: Any and all personal data provided to us will be used to ascertain your eligibility to access the services and other information herein. The collection, use, disclosure, and retention of your personal data is subject to the AIA Personal Data Policy (Singapore), to which you agree and accept. AIA reserves the right, at its sole and absolute discretion, to cancel, terminate and/or withdraw the value-added services any time or for any reason without notice.
 
The insurance plans are underwritten by AIA Singapore Private Limited (Reg. No. 201106386R). All insurance applications are subject to AIA's underwriting and acceptance. This is not a contract of insurance. You are advised to read the policy contract for the precise terms and conditions of the plan.
 
These policies are protected under the Policy Owners' Protection Scheme which is administered by the Singapore Deposit Insurance Corporation (SDIC). Coverage for your policy is automatic and no further action is required from you.
